Ghosts of the Past is a secondary quest in Velen. It only appears if Letho of Gulet was spared at the end of The Witcher 2. Choices

After the fight at the farm, Geralt can ride with Letho or go his own way. Parting ways ends the quest, and the two witchers never meet again.

In Lindenvale, Letho confronts Arnout Vester and his men and takes a crossbow bolt. Geralt can step in and kill the bounty hunters. Then nobody is left to spread the news, and Letho is still hunted. Geralt can also stay out of it. He talks the men into taking Letho's medallion as proof, and they ride off to tell the world Letho is dead.

Letho was never dead. He used a poison to fake his death, so the hunters would stop chasing him. At the end, Geralt can invite Letho to take shelter at Kaer Morhen, and Letho accepts.
