The Unseen Elder is an ancient and very powerful higher vampire who lives in a cave beneath Beauclair. Every other vampire must obey him. Geralt and Regis seek him out in Blood and Wine, in the main quest What Lies Unseen.
Lore
Higher vampires live in a strict order based on age. The older a vampire is, the greater his say in their affairs. The punishment for going against his will is harsher too. The Unseen Elder is one of the oldest of them all. He has lived in his cave in Beauclair ever since the Conjunction of the Spheres left him there.
The oldest vampires do not like company, and the Unseen Elder hates visitors more than most. Even Regis trembles at the thought of disturbing his peace. The Elder has no patience for unexpected guests, and he is deadly danger in the flesh.
Story
Dettlaff's vampires have overrun Beauclair, and Geralt and Regis need a way to force Dettlaff to meet them. They can seek the help of the Unseen Elder, or they can try to find Syanna instead. To reach the Elder, Geralt first needs Orianna's help. In Blood Simple, she gives him the location of the lair and a key to its entrance.
In What Lies Unseen, Geralt and Regis go to the lair together. Geralt has to find an offering for the Elder inside the caves. The meeting goes badly at first. In a moment, the Elder freezes Regis where he stands. He then seizes Geralt and throws him to the floor of the cave like a rag doll. Geralt is badly hurt.
Every word counts in the talk that follows. If Geralt wanders off the subject more than once, the Elder kills him for wasting his time. In the end, Geralt convinces him to help, though it nearly costs him and Regis their lives. They leave the cave alive and unharmed, if a little shaken. The Elder promises that Dettlaff will soon meet them at Tesham Mutna. Geralt is relieved that the Elder will not be the one he faces there.
Relationships
Regis is a younger higher vampire who fears and respects the Elder. Orianna knows where his lair is and holds the key to it. The Elder's word is enough to bring Dettlaff to Tesham Mutna.
