Extra Effect is the final perk of the Enchanting tree in Skyrim and the only one requiring skill level 100. It allows two enchantments on the same item instead of one.

Two Routes To It

It is the one perk in the tree with two possible prerequisites: Corpus Enchanter on the armour line, or Storm Enchanter on the elemental line. Only one is needed. Both cost two perk points beyond Enchanter, so pick the line whose bonuses you actually want.

The Restriction Nobody Reads

The second enchantment can only be added **at the moment of enchanting**, using a single soul gem for both. An item that already carries an enchantment can never be given a second, no matter how high the skill. That includes anything you enchanted before taking this perk.

In practice it means holding back your best unenchanted gear until the perk is in hand. A Daedric sword enchanted at skill 99 is finished; it cannot be upgraded afterwards.

What It Is Used For

The obvious pairings are two elements on a weapon, which stack with the matching Destruction perks, or a Fortify skill and a Regenerate effect on one piece of apparel. It is also how four-piece Fortify Smithing sets are built without giving up resistances.
