Gem of Destiny is an Epic weapon gem that keeps health, mana and stamina topped up. It marks one random enemy nearby as Marked by Destiny for a while. Killing that enemy leaves an orb that restores health, mana or stamina. The orb always refills whichever of the three is lowest.

How to Get Gem of Destiny Guide
The Drak'Dal Chieftain and the Shroud Stalker can drop it. So can the Fell Banshee, Fell Shackled Banshee, Fell Black Knight and Fell Crusader. One copy also lies on the remains of a Drak elite in the Veilwater Basin.
