In New Game +, the Legendary Forgotten Wolven Silver Sword - Mastercrafted diagram comes from reading Osmund's notes and Vesemir's note on the bookshelf platform inside the Kaer Morhen keep, and a master blacksmith crafts the sword from it.

1. Speak to the young Eternal Fire priest by the Devil's Pit signpost in Velen to start In the Eternal Fire's Shadow. He gives Geralt the key to the mine.
2. Unlock the gate at the bottom of the Devil's Pit and enter the mine.
3. Follow the mine to the doors with Reinald's fifth letter pinned to them.
4. Free Reinald from the salt stalagmites with Aard.
5. Defeat the possessed Reinald, then the red miasmal that leaves his body.
6. Talk to Reinald's spirit. He gives Geralt the Forgotten Wolven diagrams.
7. Travel to Kaer Morhen and enter the keep.
8. Climb the ladder by the bookshelves to the upper platform.
9. Read Osmund's notes and Vesemir's note on the bookshelf at the far end of the platform. This unlocks the mastercrafted and grandmaster diagrams.
