Only Pain is a side job that begins when V comes across three NCPD officers beating a man in an alley in Vista del Rey, Heywood. The victim is a wealthy corporate man, Nigel Hamilton, and the situation is not what it seems. V can stop the beating, walk away, and decide what to do with the man afterwards.
Story
To Serve and Protect
Johnny's journal entry is bitter. He says "To serve and protect" should really read "To extort and torture". He does not know what the man has done to the police, but if V does not step in, they will scrape him off the pavement. Then again, he adds, maybe it is for the best: nobody in Night City dies an innocent soul.
The three officers are Corey Aldwyn, Keegan Kelly and William Becker. V can talk to Officer Aldwyn and demand that they leave the man alone, which turns them hostile, or turn a blind eye and walk away, in which case the police kill the man and the job fails.
The Truth
A Streetkid V can ask Aldwyn whether this is how he protects and serves, and learn the story. Hamilton is a rich corporate man who showed a young woman an illegal braindance that put her in a coma. She is the daughter of one of Aldwyn's friends. Because of Hamilton's status, he was never going to be charged, so Aldwyn decided to take justice into his own hands. If V says V will deal with Hamilton, the officers leave peacefully.

However the officers are dealt with, V must then wake Hamilton and talk to him. V can let him go at once, ask him the truth, or make him pay for the rescue. If V asks what the police wanted, he claims not to know and suggests they attacked him for no reason. If V says V knows he has been showing spiked braindances, he admits that he knew they were harmful, and that he showed one to the girl on purpose.
Only Pain Map Location and Walkthrough
Only Pain map location:
- Enter the alley in Vista del Rey and talk to Officer Aldwyn.
- Demand that the officers leave the man alone and defeat them. A non lethal approach keeps the officers alive without failing the job.
- Or, as a Streetkid, ask if this is how they protect and serve, then say V will deal with the man, and the officers leave.
- Wake the corporate man.
- Talk to him and choose how to deal with him.

Choices
Hamilton's Fate
Telling Hamilton it is his lucky day, or to get lost, ends the job with no payment. With enough Cool, V can demand payment for the rescue. With enough Body as well, V can demand all of his money for a larger sum. Asking for the truth with enough Reflexes, or confronting him about the braindances as a Streetkid, gets more of his story before V decides.
V can also kill Hamilton as he leaves. His body holds more money, showing he lied when he claimed to have handed over everything.
Walking Away
Saying V was just leaving, or walking away, fails the job, and the police kill Hamilton.
Rewards
Completing Only Pain pays €$ 2,280.
The job pays a little experience and Street Cred, and eddies if V makes Hamilton pay.
