Grandmaster Ursine Steel Sword is the best steel sword for adrenaline gain. It is the Grandmaster variant of the Ursine Steel Sword, witcher gear from Blood and Wine. It deals 335-409 damage at level 40, with +21% adrenaline gain and +75% crit damage. The adrenaline gain rises to 22% in NG+. Its +14% dismember chance is the highest dismember chance of any steel sword. The diagram is on a skeleton deep in the Owl Eye Grottos in Toussaint. It is late-game gear.

The adrenaline gain suits an adrenaline build, and +5% crit chance backs up the crit damage. It also gives +5% XP from humans and nonhumans and has 3 rune slots. With the heavy Ursine set it suits a Quen tank, because both set bonuses work on Quen.

The pin marks the Owl Eye Grottos, a cave north of the Tufo Vineyard in Toussaint. Clear the rubble behind the collapsed house near the Tufo Vineyard with Aard. Loot the chest inside and read the owner's notes to learn of the Owl Eye Grottos.
Go to the Owl Eye Grottos and drop down through the cave entrance.
Kill the giant centipede at the bottom.
Blast the rock wall with Aard and go through.
Read the warning the witcher wrote, then follow the symbols on the walls.
