The Fire Elemental is one of the Elementa, a living creature of flame made by magic. Fire Elementals are found in magical ruins. One is met in a level 28 main quest in Novigrad, so they turn up in the middle of the game.
Lore
Ransant Alvaro created the first Fire Elemental. The flames burned down the whole laboratory, along with every other building on the block.
Fire is the most destructive of the elements. The creature that embodies it is just as deadly. Fire Elementals are forged in complicated magic rituals for one single purpose: destruction. They pursue it with murderous determination.
Behaviour
A Fire Elemental attacks with fireballs and streams of flame. These turn anything in their path to ash in the blink of an eye. Like golems and other elementals, it does not bleed, and poison does not touch it.
The air around it is as hot as a furnace. Simply standing close to one burns.
Weaknesses
Silver blades harm a Fire Elemental, so coat the silver sword with Elementa Oil. Leave Igni alone. Fire spells do no harm to it and make it stronger instead. Aard works against it and puts out its flames for a short time.
Strike from a distance when possible. Northern Wind and Dimeritium Bombs do it the most damage.

In Blindingly Obvious, a main quest in Novigrad, the sorceress Philippa Eilhart breaks free in Dijkstra's bathhouse. A dimeritium collar had kept her trapped in the form of an owl. Now free, she tears the bathhouse apart. In a large chamber below the bathhouse, she summons a Fire Elemental. Kill it while dodging her magic, then catch Philippa.
The Witchers' Forge, a level 30 secondary quest at Kaer Morhen, leads to an old iron mine. An Earth Elemental guards the first cavern. Further in stands a forge that speaks. An ifrit, a fire elemental of some kind, once served a mage who made special gear for the witchers of the Wolf School. The elemental kept watch long after the mage died. It rises from the forge and attacks. Once it falls, an illusion over a hidden room is lifted, and the stash of gear behind the forge is revealed.
