Tapeworm is a main job that runs alongside the second act, made of talks between V and Johnny Silverhand. It returns at the end of four main jobs: Automatic Love, Transmission, Life During Wartime and Search and Destroy. Its last stage, at a hotel in Pacifica, marks the start of the third act and leads into Nocturne Op55N1 and Chippin' In.
Story
Know Thyself
The journal sums up the job as "know thyself" turned into a two for one deal. If V and Johnny are becoming one person, Johnny suggests they keep talking. He is also happy to explain why V's only chance to survive is to reach Mikoshi in one piece. That would be a favour to him too, he says, because if V does not follow his advice, he will inherit whatever is left of V.
Getting to Know Johnny
Each stage of the job starts after one of the big main jobs of the act. After Automatic Love, it plays as V leaves Clouds. After Transmission, it plays on the way out of the chapel. After Life During Wartime, it follows the talk with Anders Hellman. After Search and Destroy, it follows the talk with Hanako. The order depends on which jobs V finishes first, so the talks happen in different places.
At first Johnny is aggressive and argues with everything. Over time, if V talks to him instead of taking the pseudoendotrizine to push him away, V learns about his hatred of the corporations and the way they shape the world for their own ends.
Hotel Pacifica
In the final stage, the Relic fails and V blacks out, waking near a hotel in Pacifica. Johnny leads V through a window to a hidden room with a cache of his old things. Before the talk ends, he asks V to help him take revenge on Adam Smasher. V can let Johnny take control of V's body for that, or refuse. Either way, V leaves the hotel through the window, the third act begins, and Johnny asks V to meet Rogue, starting Chippin' In.

Choices
The Pills
Taking the pseudoendotrizine silences Johnny for a while. Talking with him instead lets V learn more about him and shapes how the two get along.
Johnny's Request
At the hotel, Johnny asks to take control of V's body to deal with Smasher. Agreeing leads to his plan to meet Rogue. Refusing does not stop the job from being finished.
Rewards
Completing Tapeworm rewards Johnny's tank top.
Johnny's Tank Top, an Iconic piece of clothing, is obtained when the job is finished. The job also opens Chippin' In, and after the third act begins, El Capitan texts V to start the side job Freedom.
