The Spotted Wight is a Necrophage, a larger kind of Wight. Only one is left alive, in the ruins of the Trastamara estate near Beauclair in Toussaint. It is met in La Cage au Fou, a level 39 main quest of the Blood and Wine expansion. That puts it in the late game.
Lore
Spotted wights were a subspecies of wight. Witchers were said to have wiped them out. They were larger than their unspotted cousins. Their name comes from the many blotches and oozing patches on their bodies.
When witchers swear they have wiped out a species, and someone then sees one alive and well, a curse is usually to blame. This rule of thumb proved true at the Trastamara estate. The spotted wight living there was really a woman named Marlene. A curse had turned her into a monster many years earlier. It was her punishment for refusing food and shelter to a wandering beggar.
Roderick Giligan wrote a treatise on spotted wights. The treatise gives simple advice for meeting one: stay calm and leave it alone.
Behaviour
Spotted wights lived in run-down cemeteries and empty wilderness. Some moved into abandoned houses. There they enjoyed their greatest passion: brewing from their own bodily fluids.
The Trastamara wight keeps a large cauldron in the old house, and spoons lie everywhere around the ruins. The words of the curse are scratched into the walls. A spotted wight left alone is not aggressive. Threatened, it becomes very dangerous. In the cold months it sinks into a sluggish state that makes it an easy target.
Weaknesses
Coat the silver sword with Necrophage Oil before the fight. Witcher lore says that careful casting of the Yrden Sign is a sure way to beat a spotted wight, so keep the Sign ready.
A fight is only one way to deal with the Trastamara wight. Learning enough about the curse opens a peaceful path instead.

The Spotted Wight is part of La Cage au Fou, a main quest in Toussaint. Regis, an old friend of the witcher and a higher vampire, plans to brew a potion called Resonance to help find Dettlaff. One of its rare ingredients is the saliva of a spotted wight, a species thought long extinct. Yet one specimen remains alive in its lair near Beauclair.
The lair is the old house at the abandoned estate. A search with the Witcher Senses turns up the writings on the walls, the dining table and finally the wight's cauldron. The witcher then hides in a wardrobe across the room and waits for the wight to come back. If the search turned up enough clues, there is a timed choice: attack the monster or try to lift the curse.
Attacking means chasing the wight, killing it and taking its saliva glands. The kill also yields a Wight Trophy. Trying to lift the curse leads to a meal at the dining table. The right answer is to eat without the spoons. The wight's saliva is already in the brew in its cauldron, so the ingredient is collected either way. Following the stench of the brew afterward leads to Marlene in human form, and the witcher takes her to Corvo Bianco. That path later opens the secondary quest The Hunger Game. A Spoon Key in the house also starts the treasure hunt Spoontaneous Profits!.
