Phantom Great Rune is a multiplayer item in Elden Ring.
Phantom Great Rune is an online item tied to Mohg's Great Rune. An invader with Mohg's Great Rune active receives it on entering another world. Using it blesses the host's enemies with blood. Blessed enemies hit harder when blood loss happens nearby, and the invader heals when they kill a player.
Effect
The item only appears after Mohg's Great Rune is activated with a Rune Arc and an invasion begins. It is used up on use and refilled with each new invasion. The boost depends on blood loss, so it works best with bleeding weapons or bleeding enemies.
