The Spiked Caestus is a Fist in Elden Ring.
When to Use It
Mid game. It is found around Greyoll's Dragonbarrow, which opens up past Stormveil Castle, so it arrives once a build has taken shape.
Why Players Pick It
The other half of the same decision: at 60 Arcane under Blood or Occult it offers higher bleed buildup than Star Fist at the cost of lower damage. Unique multi-hit R2 shared with the Caestus, 8 Str / 10 Dex, 2.0 weight, and buyable early from the Isolated Merchant in Greyoll's Dragonbarrow.
Upgrades
Ordinary Smithing Stones carry it all the way to +25, and because it is not a Somber weapon it will also hold a grease or a weapon buff, which is a damage source the unique blades simply do not have.
Build
Its scaling runs Str at D and Dex at D, so Str and Dex are the stats worth levelling, and you need Str 8 and Dex 10 before you can lift it. Any Ash of War and any affinity will sit on it, so its scaling can be re-cut towards whichever stat you are already levelling. It also causes blood loss buildup (45) on its own, with no infusion needed.
Notes
A caestus with the rivets replaced by blood loss-inducing spikes. Fist weapons come in pair, and two-handling this weapon will equip it in both hands.
