The Poisoned Hand is a Fist in Elden Ring.

When to Use It

Shadow of the Erdtree. It is found around Belurat, Tower Settlement in the expansion, which opens only once Starscourge Radahn and Mohg, Lord of Blood are beaten.

Why Players Pick It

106 poison buildup: the highest single status figure across all five classes, plus A Arcane at max standard. Its +7.5%/20s damage buff fires on any Poison or Scarlet Rot proc, applies to spells, stacks with Kindred of Rot's Exultation, and works from the off-hand even when you don't meet its 28 Arcane requirement. Somber, not infusible.

Upgrades

It takes Somber Smithing Stones rather than the common kind, one per level up to +10, and it keeps its own skill for good: no Ash of War will replace it and no affinity will re-cut its scaling.

Skill

Poison Spear-Hand Strike costs 13 FP. Makes hand into the shape of a spear before unleashing a plunging stab that penetrates the body of the enemy. Afflicts foe with a large dose of deadly poison. It belongs to this weapon alone: no Ash of War will move it onto anything else, and nothing will replace it here.

Build

Its scaling runs Str at E and Arc at C, so Arc is the stat worth levelling, and you need Str 8, Dex 8 and Arc 28 before you can lift it. Because the skill is its own, the build has to come to the weapon rather than the other way round. It also causes poison buildup (106) on its own, with no infusion needed.

Notes

A glove stitched together from the flayed skin of the victims of a butcherous bloodbath. Afflicts target with deadly poison. Raises attack power when poisoning occurs in the vicinity. Forged of an unyielding, black impulse toward revenge fostered in those who have had everything burned or stolen from them, these are the weapons of the utterly downtrodden.
