The Sword of Milos is a Greatsword in Elden Ring. It is a mid-game find.

The Sword of Milos is a sinister greatsword fashioned from a giant's backbone, its spines tapered so it metes out wounds like a lopsided saw-blade; Milos was an undersized giant viewed as sullied and grotesque. It is dropped by the Dung Eater, either when he is killed in the Subterranean Shunning-Grounds or when he invades in the Capital Outskirts moat. It causes blood loss and restores a little FP whenever an enemy is defeated, and its heavy attack is a unique upward diagonal slash shared with Death's Poker that steps forward when charged. Its unique skill, Shriek of Milos, lets out a cursed scream that lowers the damage negation and status resistance of nearby foes, and while it lasts strong attacks become a combo. It suits a Dexterity character and works as an FP-sustaining partner weapon.

How to Get Sword of Milos Guide

It drops from the Dung Eater, either when he invades in the moat of the Capital Outskirts or when he is killed in his cell in the Subterranean Shunning-Grounds.

1. After reaching the Altus Plateau, talk to the Dung Eater in the Roundtable Hold while holding a Seedbed Curse to receive the Sewer-Gaol Key.
2. Rest at the Underground Roadside Site of Grace in the Subterranean Shunning-Grounds beneath Leyndell, Royal Capital.
3. Drop down the open grate and follow the path to the Dung Eater's cell, then open it and set him free.
4. Return to the Roundtable Hold and read the message he leaves behind.
5. Travel to the moat of the Capital Outskirts where Blackguard Big Boggart sits and walk toward its middle.
6. Defeat the Dung Eater when he invades to receive the sword; killing him in his cell instead also drops it.

It is exclusive to this weapon.
