Second Heart brings V back from death once per cooldown. When V's Health hits zero, the implant refills it to full on the spot. A fight that should have ended in a reload goes on instead. It is a late game safety net that suits any build, from a sniper who gets caught to a brawler who takes one hit too many. A Body character also gains extra Health from it for each point of Body. Second Heart is a late game implant that every ripperdoc in Night City and Dogtown sells from level 30.
Effect
Second Heart triggers on its own the moment V's Health reaches zero. Instead of dying, V is restored to 100% Health. A long cooldown then starts. At Tier 4 the cooldown is 300 seconds, and higher tiers shorten it.
The cooldown is long enough that Second Heart usually works once per hard fight. It gives V a second chance against a boss, a cyberpsycho or a large gang ambush. After it triggers, V fights on with no safety net until the cooldown ends.
Second Heart is attuned to Body. Each point of Body adds half a point of Health. Its stat bonuses add Explosion Damage and, from Tier 2, Grenade Recharge Speed.
How to Get Second Heart Guide
- Reach level 30.
- Visit any ripperdoc clinic in Night City or Dogtown and open the Circulatory System slots.

Upgrades
Ripperdocs sell Second Heart at Tier 4 from level 30 and at Tier 5 from level 40. An installed copy can be raised a tier at a ripperdoc with the matching item components. Each tier shortens the cooldown. The full Health refill stays the same at every tier. Second Heart has a high capacity cost. The Driver Update perk adds one more stat bonus. The Chipware Connoisseur perk offers a choice of bonuses when V upgrades the implant. V gains Cyberware Capacity with each level and from Cyberware Capacity Shards. The Edgerunner perk lets V go over the limit at the cost of some max Health.
The table shows what each tier step of Second Heart costs at a ripperdoc.
| Upgrade | Components | Requirement |
|---|---|---|
| Tier 4 to Tier 4+ | 50 Tier 4 components | - |
| Tier 4+ to Tier 5 | 37 Tier 5 components | - |
| Tier 5 to Tier 5+ | 50 Tier 5 components | - |
| Tier 5+ to Tier 5++ | 125 Tier 5 components | Phantom Liberty |
Character Builds
Second Heart fits any build, but Body builds get the most from it. More max Health means the full refill is worth more. Adrenaline Rush adds 35 max Health and unlocks Adrenaline from Health Items. Painkiller and Comeback Kid keep Health climbing after the implant fires.
Several perks and implants shorten its long cooldown. Cyborg in Technical Ability cuts all cyberware cooldowns by 15% when every slot is filled. The Newton Module trims cooldowns a little with every kill. The Quantum Tuner from Phantom Liberty shortens all other cooldowns and can refund a used implant.
Other low Health implants cover the moments before Second Heart fires. The Reflex Tuner slows time below 25% Health. ThreatEvac adds movement speed at low Health. Scar Coalescer adds armor below half Health. Together they make V very hard to put down.
